Incident Narrative

On March 10, 2021, an employee was trying to start a sleever machine (used to slide a plastic label over a plastic bottle). He was lifting the sleever's handle with his right arm while pushing on the lift lever with his left hand. The lift rose and about 1/4 inch of the employee's left middle fingertip was pinched between it and the lever. The quarter-inch of fingertip was amputated.
