Incident Narrative

An employee was using an extension ladder to retrieve items in a loft area in a garage. The ladder slid out from under him and he fell 9 feet to the concrete floor. The employee came down on his left side. He suffered a brain bleed; severe bruising on the left hip and buttock; a left rotator cuff tear; right arm/wrist pain; a sore neck, rib cage, and lower back; and a severe headache. He was hospitalized.
