Incident Narrative
An employee was helping to replace a tilt cylinder on a forklift. A second forklift was being used to lift the forklift that was being repaired. The lifted forklift slipped and pinned the employee's fingers between the cylinder and frame. Four fingers on the employee's right hand were amputated.
