An employee was welding a saddle onto a beam for wood beam installation. The employee was working on an extension ladder at approximately 12 feet, which was leaning against a steel column. The column's anchor became dislodged, causing the employee to fall to the ground. The employee sustained a broken right radius, which required surgery.
