Employee was elevated approximately 12 ft. on a Crown High Level Order Picker removing items from a shelf when she stepped backwards and fell to the concrete surface below. Employee was donned in a harness, but did not have it attached and secured. Employee sustained a compound fracture to her right ankle and a cracked heel.
