Incident Narrative

A temporary employee was transferring an expanded metal sheet from an expanded metal press to a leveler machine. As the employee was feeding the expanded metal sheet into the leveler machine, fingers on his left hand became caught in the openings on the expended metal sheet, and his hand was pulled into the leveler rollers. The employee sustained lacerations to his left index, middle, and ring fingers, and the ring finger was amputated.
