An employee was driving a van to deliver packages and stopped to drop off a package. The van started to move, and the employee tried to stop the van by jumping back into the van. The van rolled back into a ditch and pinned the employee's right foot between the van and the concrete culvert in the ditch, resulting in amputations to all the toes on his right foot.
