Incident Narrative
An employee was operating a pallet jack and walked to the front of the equipment to select product for an order when he stumbled and pressed the controls. The jack moved forward and caught his right ankle between the pallet jack and another one directly in front of him. He suffered multiple fractures to his ankle that required hospitalization and surgery.
