Incident Narrative

An employee was repositioning a scaffold form system to the next highest bracket on the elevator and stairway core. The scaffold system shifted and inverted while it was being hoisted by the crane. The employee fell 65 feet to the ground below. He suffered a broken leg, a pelvic injury, back injuries, wrist damage, and possible brain bleeding.
