Incident Narrative
An employee was operating a table-top laminating machine for transformers. As the employee reached beneath the machine frame to adjust the position of a wire on the transformer coil, his finger was caught in the point of operation where the metal lamination plates are automatically inserted into the transformer coil, resulting in amputation of the left middle fingertip.
