Incident Narrative

An employee was standing on top of a wooden skid, bending over and pulling up on plastic straps wrapped around a bundle of cardboard (so that a forklift could slide its forks under the bundle. The straps snapped and the employee fell backward onto the concrete floor. He suffered a compression fracture to a lower-back vertebra.
