An employee was dumping a self-dumping forklift hopper attached to a forklift. When the hopper rolled forward, it lifted the hopper skid off the forks and caused the skid to slide back, pinching the employee's left index fingertip between the skid and the fork of the lift. The employee suffered a fingertip amputation.
