Incident Narrative

An employee parked a forklift inside a trailer while unloading freight. The employee was adjusting deck bars loaded with freight when a deck bar and freight fell, knocking the employee into the forklift blades. The employee sustained a fractured jaw and lacerations on their mouth; the employee was hospitalized.
