Incident Narrative

At about 10:00 p.m. on September 3, 2021, an employee was climbing down an approximately 6-foot A-frame ladder after painting air conditioning vents on a ceiling. One of his feet got caught and the other left the ladder at the third or fourth rung, resulting in a compound fracture to the left ankle.
