Incident Narrative

An employee was pushing expanded metal material through a leveler when the glove on their left hand became caught on rollers. Three left hand fingers were crushed in the rollers. The employee required medical amputation of the index finger to the proximal joint and amputation of the middle finger to the middle joint. The left ring finger and thumb were lacerated.
