Incident Narrative

A hoisted metal cover, about 3 feet by 3 feet, fell off the hook of an overhead crane that had been used to lift it. It landed on the left hand of an employee who was cleaning under it, and he suffered fractures and a partial amputation (below the nail) to the left middle finger.
