Incident Narrative
An employee was staging pallets in front of a door using an electric pallet jack. The employee backed up and the pallet jack slid back into the door and became stuck. The employee then stepped down from the pallet jack to try to move it when it reversed and the platform caught the employee's right ankle against the concrete of the dock pit, resulting in an ankle fracture.
