At about 6:00 p.m. on July 23, 2021, an employee was using a forklift to empty a pellet hopper into a roll-off box. The hopper bounced back and the employee's left index finger was pinched between the forklift fork and the hopper's channel iron base. The employee suffered an amputation to the finger.
