Incident Narrative

An employee was providing oversight to an excavator as it was placing steel plates over a trench box. High winds caused the steel plate to torque and strike the trench box spreader. The plate bounced back and struck the employee, causing a tibia fracture, laceration to the lower leg, and dislocation of the elbow.
