On July 20, 2021, at approximately 8:00 a.m., an employee was on a 6-foot high rolling platform ladder hanging 10-foot metal hooks on a chain. The employee was hanging a hook when one of the hook ends got caught in the handrail of the rolling platform ladder and tipped it over. The employee fell to the concrete floor and sustained a contusion to their side.
