Incident Narrative
An employee was assisting a plating operator with moving a 5/8" X 60" X 125" steel plate weighing 1,200 pounds to a 4" X 4" piece of dunnage. During the transfer, the employee placed his hand on the dunnage as the operator was using a pry bar to move the plate to a table. The plate caught the employee's ring finger and crushed it, resulting in a laceration that became an amputation.
