Incident Narrative

After emptying the bed of a dump truck, an employee lowered the bed and noticed that the tailgate locking pin had become wedged. The employee used a shovel from the truck to dislodge the pin. When the pin released, the tailgate latch pinched the employee's left middle finger above the first joint, resulting in a fingertip amputation.
