Incident Narrative

An employee was unloading angle iron from a flatbed trailer located inside the facility. While traversing the trailer, the employee's foot was caught on a piece of angle iron on the trailer floor, causing the employee to fall approximately 4 feet from the trailer to the concrete floor. The employee's head contacted the floor, resulting in a head injury.
