Incident Narrative

An employee was standing within proximity of the drilling pipes as the pipes were being tightened with a wrench and the bucket of a mini excavator. The wrench that was held in place by the excavator bucket slipped, causing the drill pipe to move uncontrollably and strike the employee in the head. The employee sustained a concussion and a cut/laceration.
