Incident Narrative

During a changeover, an employee was trying to move a stand, but the actuator would not move. He then tried to use a racket wrench on the top nut to get it to engage but was unsuccessful. The wrench was left on the nut. The employee found a missing key and turned the switch that allowed the system to engage. When the system engaged, the wrench hit his right arm, fracturing his right distal humerus.
