Incident Narrative
An employee was trying to release the mechanical hold down for a dock plate while a forklift lifted the plate up so that the employee could access the area. When the hold down was released, the forklift moved from under the dock plate, allowing it to fall onto employee. The employee suffered two broken ribs, a laceration to his liver, and injury to his diaphragm.
