Incident Narrative
An employee was using a shovel to excavate roots. The blade was positioned under a root and the employee was using the curve of the blade as a leverage point when the handle snapped. The fractured end of the shovel handle struck the employee's face, causing lacerations to the forehead and eye lid, and a 1mm puncture to the eye.
