Incident Narrative

An employee was kneeling on the floor to remove a castor off of the bottom of the 2.5 gallon capping machine. The machine was sitting on a pallet about to be picked up by a forklift. As the forklift lifted the machine, the machine tilted over and fell onto the employee. The employee sustained a broken ankle, a broken foot, and a fractured vertebra.
