Incident Narrative

An employee had been pulling samples from drums, placing them in jars, and placing them in the lab for testing. He headed to lunch, then returned and began to work in the lab under the lab hood. When he opened a jar, there was a flash fire and chemicals splashed onto him. He suffered burns to the neck, chest, left hand, and wrist and was hospitalized.
