Incident Narrative

An employee was cutting on a column and began to cut into a pipe with a torch. When the torch cut through the pipe, diesel (residual fuel) splashed back onto his body. He sustained burns to his right arm, left wrist, and the right side of his face, requiring hospitalization and surgery. PPE was not worn at the time of the incident.
