Incident Narrative

On October 2, 2020, at approximately 8:45 AM, in preparation for cleaning activities, an employee crouched down beneath a conveyor belt. The employee lost balance and his left hand landed on the belt tension roller. He was pulled in between the roller and the belt, resulting in crushing injuries to the left hand and forearm. The employee was hospitalized and required surgery.
