Incident Narrative
An employee was directing crane operations. Other employees were welding an H-beam together for the ring beam. The apparatus used to twist the 30-foot H-beam failed and the beam fell off the fixture onto the employee's right leg. The leg was crushed below the knee resulting in a surgical amputation.
