An employee was climbing a 20-foot extension ladder. He fell from 16 feet up; his head struck a beam before he landed on the platform below, and he briefly lost consciousness. He was hospitalized, having suffered four cracked ribs, a cracked pelvic bone, a concussion, and a broken hand.
