Incident Narrative
An employee fell a distance of approximately twenty feet while performing concrete work on a pour form wall. One of the engineered ties used to hold the form in place snapped allowing for concrete to pour out of the form, striking the scissor lift from which the employee was working, and causing the scissor lift to topple over. The employee fell to the ground where he sustained serious injury to his arm and pelvic region.
