Incident Narrative

A 2,200-pound bulk bag of peanuts had fallen to a warehouse floor and come open. Employees were cleaning up when another bulk bag fell onto one of them, covering his entire body except for his head and feet. He suffered a broken femur and humerus, as well as head lacerations. He was hospitalized.
