Incident Narrative

On August 23, 2020, an employee was checking the pressure on compressed oxygen cylinders used for boiler chemistry control. When the employee opened a cylinder manifold valve, the stainless steel braided hose connecting the cylinder cradle to the pressure gauge failed and whipped around, striking the employee on the back and the left elbow. The employee suffered a 6-inch laceration to the elbow and was hospitalized.
