Incident Narrative

An employee was spotting a forklift as it moved a 11,642-pound steel shaft. The shaft moved, and when the forklift tines were raised it was knocked off its wooden end supports. It rolled over onto the employee's foot, crushing the top and middle parts of the foot. The foot was amputated just below the ankle.
