Incident Narrative

An employee was standing on a pallet set on top of a forklift, which was then raised to reach the store's roof (10 feet above the ground), in an attempt to mitigate water leaking on the store's computers. The employee fell 10 feet off the roof to the ground, hitting the pallet on the forklift on the way down. He fractured an eye socket, his nose, and three ribs, requiring hospitalization.
