On August 10, 2020, an employee was removing tools from a press. He was using his right hand to operate the hoist chain while his left hand was on the eye bolt. When he pulled the hoist chain, it created tension on the eye bolt as it picked up the load. His left index finger was then crushed between the eye bolt and the spinner, resulting in a finger amputation.
