Incident Narrative

Two employees were troubleshooting a lift table that was not functioning. When air flow to the table was turned on, the table tilted down quickly and an empty mold plan slammed into the guide rail. An employee's right index finger was smashed between the guide rail and the mold; the fingertip was amputated during treatment.
