A forklift was about to lower 800-pound steel decking, being carried on the forklift's forks. Instead the load titled forward and slid off the forks; it fell about 12 inches and hit an employee in the head and the ankle. The employee suffered fractures to the ankle, the tibia, and a back vertebra.
