Incident Narrative

Two employees were operating high reach stand-up forklifts. Employee 1 was traveling north along an aisle in a room while employee 2 was putting away a pallet in the same aisle. Employee 1 jumped out of his forklift and was crushed between his forklift and employee 2's forklift. Employee 1 was hospitalized with a pelvic fracture.
