Incident Narrative

An employee was backing a forklift out of a trailer when the truck's roll-up door rolled down about 6 inches. He pushed it up using his hand and pressed the accelerator. His hand was then caught between the forklift cage and the truck's roll-up door, suffering fractures. He was hospitalized.
