An employee was replacing forks on a warehouse forklift truck. The employee was standing in front of the forklift, facing the forklift carriage. He lifted the pin on top of the carriage fork holder and was installing the fork. His co-worker, who was sitting in the forklift, operated the control lever and pinched the employee's left thumb between the carriage and the fork, resulting in a thumb tip amputation.
