Incident Narrative
An employee stepped on a storm inlet grate that was not seated fully on the top flange after getting off equipment. The lid flipped in and the worker fell approximately 15 feet to the bottom of the inlet. He was hospitalized with a dislocated shoulder and possible cracked vertebrae.
