Incident Narrative

An employee was standing on a stack of cardboard, labeling and reviewing items to be moved when a stock order picker (equipment) came around the corner. When the employee saw the equipment approaching, she attempted to step down and ensure she was not in a hazardous situation with the equipment. She stepped on the side of a wheeled cart, causing the cart to move, and she lost her balance. She fell and hit her head on the concrete floor, suffering a subdural hematoma that required hospitalization.
