Incident Narrative

An employee was disconnecting the electrical supply from a piece of equipment for removal and transport. When he returned to the machine after testing the source to be sure it did not have power and verifying that the disconnect was in an off position, the cut wires burst open from an arc flash that burned his face, left hand, and arm.
