Incident Narrative
An employee was cleaning mud out of a concrete footing with a hydroexcavator when the insert from the excavator's rubber vacuum hose sheared off, severing the connection with the dig tubes and causing the dig tubes to fall and strike the employee. The employee was hospitalized with a compression fracture to the back.
