Two employees were working on a 16-inch plastic water pipe, drilling for a saddle tap when the pipe exploded. One employee suffered an amputation of the right arm, a laceration to the right leg, and the loss of vision in the right eye. The second employee suffered lacerations to the legs and sand in the eyes.
