Incident Narrative

An employee was troubleshooting the hydraulic system on a customer's skid to get the hydraulic pump to run. He started up the engine and an unexpected release of hydraulic energy occurred. He attempted to open the dump valve and the discharge hose exploded with his hand on it, releasing oil and severely lacerating his right index finger.
